Resource and Service Coordination

Day Program
Our structured day program runs Monday through Friday, offering residents a consistent, engaging routine filled with meaningful activities, skill-building, and social connection. It's a safe and supportive environment where individuals can thrive during the day while staying active and engaged.
In-House Social Worker
Our dedicated in-house social worker is here to support residents and their families every step of the way. From navigating complex systems to providing emotional support and advocacy, our social worker ensures that every individual has a trusted professional in their corner.
On-Site Medical Care
The health and well-being of every resident is a top priority. We are proud to have a licensed clinical nurse practitioner and a psychiatrist who visit the home personally each month — and a pharmacist who personally delivers medications monthly — providing professional medical oversight, mental health support, and continuity of care right where our residents live.
